Uruguay Case: Applying technology and connecting the Uruguayan Healthcare System

National Online Clinical History (HCEN) and My Digital Clinical History (HCD)

Through the National Integrated Healthcare System (SNIS for its acronym in Spanish), Uruguay has promoted a digital transformation in the healthcare sector for the benefits of its citizens, and the Salud.uy initiative was created with the goal of prioritizing the intensive use of information technology tools (TIC for its acronym in Spanish) within the sector. One of the main pillars was the creation of the National Online Clinical History (HCEN for its acronym in Spanish).

Requirements

initiative was created with the goal of prioritizing the intensive use of information technology tools (TIC for its acronym in Spanish) within the sector. One of the main pillars was the creation of the National Online Clinical History (HCEN for its acronym in Spanish). Within the National Online Clinical History components is the development of a user healthcare system portal, which allows for them to access their corresponding digital clinical information. Therefore, citizens have equal access to their healthcare information, which leads to greater transparency regarding clinical information, to a higher-quality level of care, and to an improved healthcare continuity for the process.

Solution

The platform to support the National Online Clinical History was developed, where the different providers in the National Integrated Healthcare System (SNIS) enter the clinical information for their users in accordance with the adoption plan defined in the Salud.uy programme. This is based on interoperability and allows the exchange of clinical information between different healthcare providers for welfare purposes, regardless of where the information was generated.

The platform has the following characteristics:

  • A major architecture, which integrates clinical information (studies and other consultations) based on CDA documents.
  • It includes interoperability standards and mechanisms.
  • It is compliant with the current regulations set forth by the regulatory authorities.
  • It is compliant with information security criteria.

The citizen access to their Online Clinical History is made possible through the use of the “My Digital Clinical History” (my HCD), which can be accessed by previously verifying the user against the defined security tool. Therefore, citizens have access to their digital clinical information, where they can configure the different healthcare providers who can access their clinical information, and perform traceability regarding who accesses their information, within which framework, and why they are accessing it. 

Results

95% of citizens have an Online Clinical History. 
My HCD is available for all citizens older than 18 years of age.
