In Uruguay, now everyone has a Digital Health Record

Salud.uy began its activities in 2012 under the framework of the Agency for Electronic Government and the Information and Knowledge Society (AGESIC) of the Uruguayan government. The purpose of this program, co-governed by the Presidency of the Republic, the Ministry of Public Health, the Ministry of Economy and Finance, and Agesic itself, is to integrate information and communication technologies into the Sistema Nacional Integrado de Salud (SNIS), the National Integrated Health System. The program is jointly funded by the Uruguayan government and the Inter-American Development Bank. 

Its main task is the development of the National Electronic Health Record (HCEN). It is based on a federated model of digital health records that are stored in the healthcare service providers chosen by the system user to receive assistance. The State provides the interoperability platform that makes its operation possible with the necessary guarantees, adopting international computer standards.

The Challenge

Before Mi Historia Clínica Digital, a minority of SNIS users could already access all or part of their digital clinical information because their healthcare providers offered this service. However, the vast majority of the population did not have access. Therefore, the primary foundation of Mi Historia Clínica Digital is to provide equal access to clinical information for all Uruguayans

Healthcare organizations that did not have a user portal, including the country's primary public healthcare provider, immediately benefit from this universal solution provided by the State.

For the production deployment of MI HCD, Salud.uy and GeneXus Consulting tackled multiple technological challenges over the course of four months. First, it was necessary to analyze and define the product's architecture on the HCEN platform. It's worth noting that Mi HCD is the first application developed and made available to the public by Salud.uy through the Internet, so there were no prior precedents for a project of this nature on the platform.

Additionally, it was necessary to integrate with the user authentication methods provided by the Uruguayan government through the Electronic Identification area of AGESIC. This includes the Digital ID and Mobile Digital Identity.

Finally, the study of the user interface design required the collective expertise and knowledge of both parties, both at the national and international levels, to arrive at a successful and highly satisfying product.

The Solution

Through the Mi Historia Clínica Digital (My Digital Health Record) application, as a user of the Sistema Nacional Integrado de Salud (SNIS) and being over 18 years old, it is possible to access your own digital clinical information and configure permissions and privacy settings from any computer, tablet, or mobile phone.

Furthermore, the tool allows for the configuration of access privacy policies, meaning the selection of healthcare providers that the user will enable to access their digital clinical information through the HCEN platform for an unlimited timeframe. 

Finally, it displays the access history to the user's clinical information by healthcare professionals from various providers, or the user themselves. Authentication is done through their Digital ID or their Mobile Digital Identity; both options allow access with the highest level of cybersecurity established by AGESIC.

The Work Team

The program's executive team, along with healthcare ecosystem experts, began conceptualizing this application in 2017. They considered previous experiences from other countries, as well as local initiatives from some of the healthcare providers. By early 2019, it was understood that the conditions were in place to open access to the owners of clinical information, namely the users of SNIS. To do this, an interdisciplinary work team was formed, which included program members along with technical experts from GeneXus Consulting. The excellent collaboration between both parties was a key factor in the project's success.

Additionally, an external group of experts was called upon to act as advisors throughout the project.

The GeneXus Consulting team made a significant contribution to the project's success, providing the most qualified technicians with extensive dedication and commitment throughout the entire work process. This was evident both in the development of the solution and all the challenges that arose in the process, as well as in the study of interface design.
